Notification Form
|
- A separate notification shall be submitted for each item covered by different standards and for each invoice.
- The notification form shall accompany a copy of the Conformity Certificate indicate conformity of the consignment to the relevant Sri Lanka Standard Specification.

Copy of Commercial Invoice. |

Copy of Packing List giving batches/designs/sizes included in the consignment and quantity from each batch/design/size.

Copy of Bill of Lading. |

Copy of Customs Declaration with customs entry number. In case, if the importer is not in a position to submit custom entry number at the time of submission of the notification, bank stamped invoice shall be submitted |

Acceptable Conformity Certificates (CC) if any. If the consignment consists of several batches and different design, quality certificate shall be provided for each batch and design. |

Step 1
|
Submit the duly filled Notification Form
|
Step 2 |
SLSI will decide whether the consignment should be subjected to inspection or sampling and testing or not.
|
Step 3 |
If sampling/inspection is to be carried out, Importer shall pay the cost applicable prior to inspection and sampling is undertaken. Then Importer shall inform to the SLSI, date of inspection with time and location.

Step 4 |
The officer(s) of the Sri Lanka Standards Institution will conduct sampling in accordance with the relevant sampling procedures.
Selection shall be done at random, using random number tables specified in SLS 428.

Step 5 |
If the sample examined is found to be sub-standard on critical requirements, the importer will be advised to return the consignment to the exporter.

Step 6 |
If the consignment meets the critical requirements identified by the SLSI, the notification form will be stamped as approved.
