Submit the following documents to DoC. CUSDEC, Invoices and other supporting documents.

If DoC approves the documents you may go to next step.

obtain a form from the counter, fill it and give it to the counter with an ID Copy in order to request a blank GSP CoO
|
If 02nd step is done DoC will issue a Blank CoO
|
|
3
|
Fill the blank CoO and submit it to DoC with other relevant supporting documents. (CUSDEC,cost sheet, material sheet, affidavit, bill of lading, phytosanitary certficates, goods dispatch note, if applicable)

All the applicable documents should be in order
