1	Copy / ies of licence/s issued to the user / s
2	A letter from user giving consent for export of the radioactive material / source or equipment containing radioactive material / source.
3	Special form certificate of the radioactive source, if source is to be transported under special form.
4	Package Design Certificate & Sealed Source Certificate.
5	Consent from the consignee for receiving the particular source/material.
6	Copy of Business Registration Certificate of the consignor. (This will not be applicable to Government Institutes).

Process Steps

Step 1	Applicant should submit a duly filled application form together with aforesaid required documentation and forward it addressed to the Director General of Sri Lanka Atomic Energy Regulatory Council.
Step 2	Atomic Energy Regulatory Council reviews application and if it’s not complete informs applicant .
Step 3	After Documents are assessed by the Council and the payment of inspection charges by the applicant the Council carries out an inspection of the equipment.
Step 4	Decision taken by Sri Lanka Atomic Energy Regulatory Council on the application is conveyed to the applicant within 10 working days on receipt of all requested information to assess the application
