2	Provide the Formula Number which is as shown in the manufacturers records.
3	Provide the Full H.S. Classification in correspondence to the formula number.
4	Provide the details as to the Raw Natural Rubber Content as a percentage of the weight of the end product.

Step 1	Applicant should submit a duly filled application form together with the relevant documentation and forward it to the Rubber Development Department .
